Eligible if any of the following apply and the couple has charted ≥ 6 months of fertility‑focused intercourse:

1. Infertility History:

- More than 6 cycles of fertility-focused intercourse without conception.

- 1 year of random acts of intercourse without conception.

- Couples trying ≥ 6 months without conception, with no prior fertility surgery or last fertility surgery ≥ 12 months ago and still not pregnant.

2. Gynecological or Reproductive Conditions:

- Pain with intercourse.

- Pain with menses.

- Endometriosis (diagnosed or suspected).

- Need for tubal patency assessment (e.g., suspected blocked tubes).

- Isthmocele (cesarean scar defect).

- Fibroids impacting fertility.

- Blocked fallopian tubes (diagnosed or suspected).

- Abnormal bleeding patterns:

- Intermenstrual bleeding.

- Chronic endometritis or tail-end brown bleeding.

- Polycystic Ovary Syndrome (PCOS) with surgical evaluation needs.

3. Referral for Surgical Assessment:

- Referred by another physician, advanced practitioner, or FertilityCare Practitioner (FCP) for surgical evaluation.

Eligible Patients:

Schedule directly with Dr. Hemphill for surgical consultation.

Non-Eligible Patients:

Patients not meeting the above criteria should be scheduled with a medical consultant for initial evaluation before consideration for surgical consultation.
